Could not get the guy to go outside to get a proper screenshot of the skin, but we pretty much know what it looks like just looking at Ghostly Hero.
Any more pictures / information as to where this sword drops (HoH?) would be appreciated.

But to answer one of your questions it apparently drops from the end chest after you clear the UW.
